Important Patient Notice
As from Monday 21st June 2021 eConsults will be switched off at 6.30pm every evening until 8.00am the next day - on Friday evening they will switch off at 6.30pm and resume 8.00am Monday. Whilst you will be unable to submit an eConsult when switched off access to the self - help and pharmacy advice will still be available.
Please do not contact the surgery with regards to an eConsult that has already been submitted as all eConsults will be responded to by 6.30pm the next working day for Clinical advice and within 3 working days for administrative advice. However if your condition worsens since submitting your request then please contact the surgery on 01925 631132 or ring 111.

Important Patient Information
Following extensive trials, the first safe and effective COVID-19 vaccine has been approved in the UK and is now available to priority groups.
Vaccinations are being delivered according to priority groups identified by the Joint Committee of Vaccination and Immunisation (JCVI). The vaccinations may not take place at your GP practice, but rather a local vaccination service (led by GPs, practice nurses and community pharmacists) who are responsible for delivering the vaccine to people in your community.
When it is the right time for you to receive your vaccination, you will receive an invitation to come forward. This may be via the phone, or through a letter either from your GP or the national booking system.
We know lots of people will be eager to get protected but we are asking people not to contact us to get an appointment as you will not be able to get one until you are contacted
HOW YOU ACCESS THE PRACTICE HAS CHANGED
Due to the Coronavirus Pandemic, the way you access the Practice has changed. We are trying to safely resume as many of our services as possible.
The surgery is still operating Monday to Friday from 8 am to 6.30 pm. If you have an urgent problem during these times please ring the surgery. At all other times ring NHS 111
We are currently operating telephone triage, whereby when contacted by a clinician they will review your problem and will then, depending on your needs:-
- Offer you advice via text
- Offer a video call
- Ask to see you in person (face to face) or
- Navigate you to other services
For non-urgent problems, queries or sick note requests please use eConsult (accessible via our surgery website www.greenbanksurgery.com ); you will receive a response from the surgery by 6.30pm the next working day
If advised to come to the surgery as the main doors are closed you will need to press the intercom button to the right of the entrance doors to gain access. You will be required to wear a face covering, have your temperature checked and asked to sanitise your hands before entering the building.
Please be advised that you should NOT ATTEND THE SURGERY IF ANY OF THE FOLLOWING APPLY TO YOU:
- You have a temperature (this means you feel hot to touch on your chest or back)
- You have any symptoms (a new continuous cough or a loss or change to your sense of smell or taste)
- You or a family/household member have had a recent positive Covid-19 test or are AWAITING the result of a Covid-19 test
Our reception seating area is set to ensure patients attending surgery can safely socially distance whilst waiting for their appointment and the screening at our reception desk has been added to protect both our patients and our staff.
Advice for the Clinically Extremely Vulnerable
The guidance for the clinically extremely vulnerable has been updated in line with what was announced on Monday 22 June. You are no longer advised to shield and you can now visit shops, places of worship and other venues. You can return to work if your workplace is Covid-secure but should carry on working from home wherever possible.
You should maintain social distancing wherever possible and adopt other measures to keep yourself safe, such as regular hand washing. More information on how to keep yourself safe can be found here.
Please be aware that local advice may change if incidence rates and transmission becomes a concern. The government will write to those affected when advice is changed.

More places where you must wear a face covering
From the 8 August, members of the public must wear a face covering, by law, in the following settings.
Face coverings are also needed in NHS settings, including hospitals and primary or community care settings, such as GP surgeries. They are advised to be worn in care homes. Individual settings may have their own policies and require you to take other measures.
Important Patient Notice
As from Monday 15th June 20 any patient attending the Practice should provide and wear their own face covering, this should not be a clinical grade mask but something as simple as a scarf or bandana to cover your mouth and nose whilst allowing you to breathe comfortably
IMPORTANT -COVID 19 UPDATE
We are now operating controlled access to the surgery via the intercom system only; this is to protect patients and staff at this difficult time. All routine face to face appointments have been cancelled in line with Public Health guidance with regard to the corona virus situation. Online appointment booking of appointments has also been suspended.
Please do not come to the surgery unless a member of staff has told you to attend.

HOW TO BOOK AN APPOINTMENT OR COLLECT A PRESCRIPTION HAS CHANGED
YOU WILL NO LONGER BE ABLE TO MAKE APPOINTMENTS AT THE RECEPTION DESK UNDER ANY CIRCUMSTANCES
If you wish to make a same day appointment for an acute condition or a routine GP appointment, you can complete an eConsult. The eConsult system can be accessed by clicking the 'get started' tab in the box below. It is an easy to use system and you do not need to be the registered patient to access it. A doctor will triage your request and let you know the most appropriate course of action; this may be telephone advice, a face to face consultation, a prescription or referral etc. Please be vigilant for a telephone call or text reply. You may also still call the surgery to make an on the day appointment.
If you wish to make a routine appointment for a review such as Asthma check or Diabetic check with the Practice Nurse these appointments can still be made over the telephone.
We have temporarily suspended the online booking system as all appointment requests must be triaged due to ongoing concerns around COVID-19 (Coronavirus). This is to ensure that we are able to fully assess risk before appointments are booked at the surgery. Patients are advised to seek advice via our e-consult service which can be accessed from the 'GET STARTED' button below
WE WILL NO LONGER BE OFFERING PRINTED PRESCRIPTIONS; THIS IS TO AVOID PATIENTS COMING INTO THE SURGERY UNNECESSARILY.
Patients must select a nominated pharmacy for the electronic prescription to be sent to. If you do not already have a nominated Pharmacy, please ask reception staff or contact the Chemist of your choice and advise them you would like to nominate them.
FIT NOTES/SICK NOTES / MED3
Any Fit Notes/Sick Notes/Med3 forms issued by a GP will no longer be available to collect from the surgery; these will now be sent out in the post to patients
IF YOU ARE SELF ISOLATING OR IN QUARANTINE DUE TO THE CORONA VIRUS YOUR DOCTOR WILL NOT BE ABLE TO ISSUE A SICK NOTE.

If you have been asked to have a blood test please COLLECT your blood form from the surgery where possible. If your circumstances do not allow for this we will POST the blood form to you.
Extended access appointments Pre-bookable evening and weekend appointments with a local GP are available from 6pm-8pm Monday-Friday, 8am-4pm Saturdays and Sundays 10-2pm at Bath Street Health and Wellbeing Centre. Easily accessible, the Health and Wellbeing Centre is in the town centre and has links to public transport on the town’s main bus routes. To access an out of hours’ appointment or to sign up for GP online access please call your own GP who will arrange the appointment for you. Please be aware that you will need to consent to share your medical record with the service.
GP out of hours service The out of hours GP service is open Monday to Friday 6.30pm - 8.00am and weekends and public holidays 24 hours a day. The emergency GP out of hour’s service in Warrington provides urgent healthcare services for patients who feel their illness cannot wait until their GP surgery re-opens.
